It includes racial or ethnic origin, political opinions or associations, religious or philosophical beliefs, trade union membership or associations, sexual orientation or practices, criminal record, and health information

Personal information is collected by Geena Cheung for the primary purpose of providing the services requested by individuals and organisations. When collecting personal information, Geena Cheung will disclose the type of information, the purpose of its collection, and the intended use, where possible.

Additionally, information regarding plan managers, care coordinators, and other allied health professionals involved in an individual’s care may be collected for care coordination and clinical handover purposes, with consent.
When services are provided under a contract with an organisation, Geena Cheung will comply with the relevant organisational policies and procedures related to care coordination, clinical handover, and documentation.
For documentation purposes, Geena Cheung may store personal information in practice management software, including sensitive information obtained through service provision or from allied health professionals.
To address enquiries, Geena Cheung may use the name and contact details provided by individuals or their referring third parties.
Depending on the circumstances, the following individuals may act on behalf of the individual for the purposes of this privacy policy:
- A guardian, parent, carer, or other person responsible for the care of the individual
- Someone with a general Power of Attorney or a Power of Attorney that includes health-related powers
- A person recognised under law as responsible for any aspect of the care or welfare of the client
- A person nominated in writing by the individual while they have the capacity to provide informed consent
Geena Cheung does not use personal information for marketing or promotional purposes unless explicit consent has been obtained.
Incomplete or Restricted Personal Information
Individuals have the right to restrict or refuse the collection of personal information. However, if personal information is incomplete or restricted, Geena Cheung may not be able to provide services or respond fully to enquiries, leading to delayed or insufficient service provision.
How is personal information collected?
Personal information is generally collected directly from the individuals through interactions in person, email, or online through www.geenacheung.com. In some cases, personal information about an individual may be collected from a third party, such as a family member, carer, guardian, or through a referring health service providers. Third parties providing personal information to Geena Cheung affirm that consent has been obtained for the disclosure of that information. When services are provided under a contract with an organisation, organisational policies for information collection will be followed.
Maintaining Quality of Personal Information
Geena Cheung takes reasonable steps to ensure that the personal information collected, used, or disclosed is accurate, complete, and up to date. If information is found to be outdated, inaccurate, or misleading, individuals should notify Geena Cheung promptly for correction of records to ensure quality of services.
Management of Sensitive Information
Sensitive information is only collected by Geena Cheung when reasonably necessary for operational purposes and with explicit consent, unless required by law or for the establishment, exercise, or defence of a legal claim. Sensitive information will only be used and disclosed for the primary purpose it was collected, or for related secondary purposes with consent or where permitted by law. When services are provided under a contract with an organisation, relevant organisational policies regarding sensitive information management will be followed. Individuals may use the contact details at the bottom of this Privacy Policy to withdraw their consent. When consent is withdrawn Geena Cheung will no longer be able to provide services.
Storage and Security of Personal and Sensitive Information
Geena Cheung takes reasonable steps to secure all personal and sensitive information from misuse, interference, loss, unauthorized access, modification, or disclosure. Information is stored securely in Cliniko, a practice management software that complies with the APPs and employs two-factor authentication and encryption. Any physical data is entered into Cliniko or stored on a password-protected hard drive, then securely destroyed. Information such as photos and audio recordings may be stored using cloud services (e.g., Dropbox), with appropriate security measures in place. Information is retained only as long as necessary for the purposes for which it was collected or as required by law. Please note that all reasonable precautions are taken, no method of transmission over the internet is completely secure. Transmission of personal information via this website, including through contact forms, is at the individual’s own risk.
Individual Access to Own Personal Information
Individuals have the right to access their personal information collected by Geena Cheung. To request access, please use the contact details at the end of this Privacy Policy. Requests for access may be denied if required by law or where the personal information may relate to existing or anticipated legal proceedings. Geena Cheung will respond to the request and set out the reasons for refusal in writing.

- University students on placement with Geena Cheung,
- Other allied health professionals involved in your care,
- Plan managers, care coordinators, and in formal reports when requested, and
- As required or authorized by law.
Informed written consent will be obtained prior to the disclosure of any personal information, unless otherwise required or authorised by law.
Data Breach Notification Scheme
If a data breach is suspected, Geena Cheung will assess it under the Notifiable Data Breach (NDB) Scheme. If an eligible data breach is determined, individuals will be notified as soon as reasonably practicable. If individuals suspect a data breach has occurred, please contact Geena Cheung immediately so an assessment as required under the NDB Scheme can be conducted.
